Ranked 3's matches not in team history / No LP change

Has anyone else seen issues this season with random games not being counted against your team or you not being credited with a win? We have seen at least 4 matches this has happened with, sometimes it benefits us and sometimes it doesn't, but it's getting really old. I'm surprised I haven't seen other people reporting this before now.
