Game crashed after queue accept

I was on with playing with a friend and after waiting through the queue, I hit accept. After I hit accept my game crashed and dumped me out of champ select. My friend said that the chat reported that I had left the lobby for the game. I had no option to reconnect to the game and got a 5 minute dodge penalty. My friend has also had this happen to him in ARAM apparently he forgot to select a champion.
