CHAMPION SPELLS NOT CASTING: Holding down QWER while respawning may cause abilities to not cast

I'm reading multiple people on this board mention something similar - While playing VI, both my Q and my R would not work, I had to die for it to work again.

I think the problem may be related to holding down QWER while respawning and or clicking to move your champion really quickly on the world or minimap?

Happened to me while playing pantheon and he has no hold down abiilities. Just everything stopped from recall to summoners. I relogged and died but they still wouldn't work with quickcast or clicking on them.
