[Gameplay] Sion abilities stop activating randomly in match

This is the third time in five ARAM matches playing as Sion where my Q and R have stopped responding. I cannot activate with clicking or using the hot key, I have tried rebinding to other keys and still cannot use these abilities. After a disconnect/reconnect they work again. VERY frustrating.

Note these abilities are not on cool down though some times I have died during use and the icon will remain lit this was not the case for any of these errors.
