Attempting to Reconnect every game and Memory Dump Hard Crash

I've been playing League for a couple of months now and approximately three patches ago i started to get random disconnects in every game. Bots and Normals and now Ranked. I stay in game and i can still hear voice chat in game or on Discord but the screen freezes and i walk back to base until i reconnect. Also, approximately two patches ago i have occasionally had hard crashes to desktop followed by an error box saying something about a "memory dump". This is all real frustrating as my computer works perfectly with all other games and programs. Is there anything i can do on my end in settings or is there some incompatibility with a piece of my hardware??? I have seen other posts on this topic and none of the proposed fixes in the comments worked. Any help would be greatly appreciated!
