While in Death Defied, Requiem is greyed out for the full period of undeath, meaning R can't begin channelling before true death. Cooldowns were all available, Karthus's level was over 6, at least one point was in Requiem.

That's because his ult channels. If you dont have time to channel the ult you cannot cast it.
