[GAMEPLAY] Both Kayn forms go on CD

On Kayn, when I unlocked my form instead of giving me a form it put both of them on CD and when the timer for them got to 0 it put them both back on a 5 minute timer again but with a 0 in-front of it (e.g. 0-5:00).

I am unsure on how to recreate it, but I think it may have been caused by me having an even number of both types of souls when it was unlocked.
