[Gameplay] Vi's q stops working

I have seen this happen twice now, I'm not sure what causes it. Once was in an ai game, once in blind pick unranked. All of a sudden pressing the q hotkey and clicking the icon do nothing. This makes her completely unplayable. I think there may be some connection to using her ult, but I can't say for sure.
