Consecutive Shut Down Bug

So unfortunately I do not have good documentation of this bug, but I decided to mention it anyway so there can be some starting documentation on the bug. I will search for a replay later and see if I can find it. Ive only seen one instance of this playing Malphite Top lane against Irelia. Irelia tower dove me I we both die (me first) and she was Shut Down. We both come back to lane she dives again, we both die she is Shut Down again. Maybe the order the deaths we're displayed was incorrect and she technically had a 2 kill streak before dying? Other than that I have no explanation for the double Shut Down.
