Permanent Valor form [GAMEPLAY]

In a recent game I was playing Quinn and had been Yorick Ulted. While a ghost, I activated Valor form and died while still in it. For the rest of the game I was stuck as Valor without the buffs that valor form normally gives. Quinn's passive remained, but the ult did nothing. Neither activating my ult nor dying turned me back. I know people don't usually play either of those champs, but it is really unfortunate to be completely locked out of the majority of your character. It was a game ending bug.
