[Gameplay] Bugged Yorick Omen of War Ghost

I was Yorick and played an ARAM. During this ARAM, somehow my Omen of War ghost bugged out, and became permanent and untargetable. He would follow champions if I was nearby and attack them, attack inhibits, minions and turrets. The enemy team couldn't kill him as he was untargetable, even to enemy turrets ( I did not try the one at the very end guarding their base). My dying did not make him disappear either.
