I had this Fiora bug on 9/7/2015 patch 5.17 , hope that dear Riot explain what was it or fix it ^-^ . and sorry for the quality and sound .

Yeah I saw this. I also had a different bug (or I think it is) where I sometimes Q into an enemy champ exactly in the direction of the vital and it misses it. I thought it prioritized the vitals. Also I sometimes Q and it walks towards her original position.
