Zac HP bar not seen after he returns from passive

Happened twice in this game. After I killed Zac and then tried to kill his passive as Ahri, he would come back with no hp bar. Others in the game could see it too. The image is kind of bad but he's attacking the tower. Another time he came back with no hp bar either but with a shrunken model.
