[CLIENT][CRASH] Graphical Freeze during Champion Select, Audio Continues

During champion select normally just before my turn to pick the client freezes, while the audio and the rest of my computer continues. I am able to click and interact with the client, but the image never changes. Because of this I cannot see who I am picking or my masteries and runes. After the initial freeze, if i am able to pick a champion just by who was originally available and showing when it froze and clicking on them the whole client crashes upon the entering the game forcing me to relog and join late. During the freeze I am able to type into the chat box and hit enter to communicate the freeze to my teammates but I cannot tell if i have misspelled anything or what their responses are because the client appears completely frozen.

This happens EVERY time, and I say normally before my pick sometimes it happens during my pick as well though it seems to consistently happen while mousing over champion portraits on the selection screen. I have re-installed twice on two different hard drives. Running windows 8.1 64-bit at 4K.

The game is completely unplayable as I can never be certain of who I am picking, what my masteries are and am completely unable to effectively communicate with my team. Also, I have to relog on average 5 times until the client actually connects to the chat server. Yes, I have exceptions made in my firewall and all recommended ports forwarded.
