I bought the second and third upgrade at the same with Viktor and exited the shop screen. I upgraded E and tried to upgrade W, but I only got the one upgrade.

Play Viktor
Get 2000 or more gold
Buy two augments of Hex Core at the same time
Augment skill

Just did this in Aram... If you buy Tier 3 before buying tier 2 you only get 2 Augments instead of all 3.
