[GAMEPLAY] Anivia Passive only working once

I recently played an ARAM game and i was playing as Anivia. My passive Rebirth only activated one time and after that the icon never came back letting me know that it will get off cool down and it never appeared again. The passive never came back and it ended up with me getting way more deaths than i shouldv'e had. Anyone else have this issue? And if there is a fix please post below.
