Katarina button malfuntion [GAMEPLAY]

When playing as katarina , after a fight i wasnt able to use her Q, R or even the recall button... Only thing that happened before was i ulted fiora and then she ulted causing my ult to stop.... After that i was no longer able to use the mentioned buttons.. This has happened to me about 5 times that i can recall... sometimes they come back online and others they stay off causing my team to lose the match.

But.. when fiora ults you while ur ulting, your ult doesnt stop. It keeps going.
