Champion Select Glitch

Me and my friend were in quene and u know a match accept came up we accepted and as soon as everyone accepted the screen was black. Me and my friend didn't dodge and we waited and after 90 seconds it said me and my friend (Ashisoji Jizo and atlasp2) didnt pick or whatever and we got a 6 minute penalty. We personally don't care about the penalty we just were saying and wondered if it was a one time thing or has been occurring lately.
