Items Disappearing??

Put an item on Neeko mid fight. She died and came back to the board after the fight with no item. It never showed up. Also picked up a Qiyana from the Carousel and it came back to my bench as as different unit and disappeared.

I've played lots of games and this is the first time but it's not the first time I've heard people talking about it since the patch.
