I've attached an imgur album with two screenshots of the end-game. There were my only two screenshots, as I was shocked when it happened.

I was about to win the game, note the 7-game win streak. My opponent decided to leave the lobby as he only had 6 hp left (and I had 54 hp), after he lost 3 games in a row as the final 2. Surprisingly, he had some great RNG this round and managed to win the round with 3 champions alive (dealing 1, 3, and 8 damage as you can see in the screenshot). Unexpectedly, my health immediately dropped from 45 to 0, and the game was over.

The image you posted is broken, also this is likely a visual bug not an actual loss in the game. Remember your place is decided once you are removed from the game. So if you are the only one left then it is only possible to get first place. In the worst possible case where the person leaving received a higher place then you. The worst case would be second place. Technically anything below 2nd place is a loss, however you only lose LP if you are below fourth place and only get a victory screen if you are second or first place.
