Mage's Cap seems to be not working

Hi, I've played last game with 6 mages and decided to give Mage's Cap to Annie. However her spell was casted only once. So I've decided to use it on Amumu instead, still spell casted only once. There are more reports regarding double casting with Mage's Cap. Can you please take a look at it?

I was playing Mage Thresh and it worked just fine

I just played a game with both mage annie AND amumu. Worked fine Did you have 3 mages? Only a 50% chance.

My gf was just using mage hat amumu with 6 mages and he only ultted once. Happened on multiple rounds. Luckily she still won.
