Fully completed item removed when combining with Neeko's Help

I had a 1-star Olaf with 3 completed items, grabbed another Olaf from the carousal with a NLR, and used Neeko's Help to combine them. 1 of the completed items popped off while the NLR stayed on instead. Also had a full bench if that makes a difference.
