Lunar bug on PBE

So I decided to try out Lunar Origin on the PBE today and encounter a bug. If you got both Leona and Karma, when the Origin supposed to activate, instead of giving the intended stats, it just render every one on your team invulnerable (HP bar is set to 0) and using the 3 stars model/3 stars health bar. The only way is to let it time out, but this is unplayable.

Also one more thing, I noticed you can't equip small item (Rod, Vest,...) while the champion is in combat anymore, full item works though. Is this intended? because I haven't seen it mentioned anywhere by the TFT devs.
