Luden's Echo only procs once per spell cast

Luden Echo's effect in TFT doesn't proc "whenever the holder deals damage with their spell" but "whenever the holder deals damage with their spell once per spell cast". I know patch 9.22 added this modification, but it has to be referenced in the tooltip, as the item either becomes a SSS or a F+ tier item depending on what it actually does.
