I think before anyone gives you advice it might be better to explain what exactly you are doing that is toxic, there's a difference between being upset and going ape **** crazy on your teammates.
learn that you can carry if you are good enough so that you blame and get mad at yourself which is actually the real problem in the first place.

I was never really toxic but I didn't used to have a good attitude in game, I would just not say anything all game, but stay sour about all of the things that happened.
Then I found the Trinity Force Podcast, they touch base a lot on your attitude and mindset while in the game, give solid tips into how you should react and how to remain positive and boost morale of yourself and your teammates. With a good attitude you'll find that you come back from a lot of those rough games.

You think I'm joking, but as soon as someone starts to irk you, mute them. Them not talking to you equals you not paying attention to their bs. Also, if you're watching them do something stupid, don't get mad and type nasty things. It takes you're focus away from the game. Don't type unless it's to try and direct the team, or to say "gg", "gj", "thnx", etc. if what they're doing is making you mad, do one of two things. Either take it as a lesson, or ignore it. Watch what they do. See what they're doing wrong, what led to them doing it wrong, what they could have done to prevent it, everything. Remember it. Make sure you don't make the same mistake. That, or you could focus on your gameplay rather than theirs. Even if you are 15/1/17 as zed, you have one death. Make sure you don't get another one instead of watching thresh pull himself into a 1v5 while the enemy is goon squaring bit lame again. Also, see why your team mates are doing bad. Are they bad? Or is it your fault in some way they did bad? Do you not Mia? Did you not help during a skirmish? Even if it hasn't been your fault, focus on making sure it isn't. Anything but letting yourself get angry at the other players. If anything, get mad at the enemy players rather than your team. Just, in the long run, turn and put your energy into something productive rather than getting mad.
Download replay software (I recommend SkinSpotlights Replay) and watch your replays. Not only will you see all the mistakes you are making, you also might get some insight into why your teammates did what they did and you might see some good plays that they made. I find most people are toxic because they can't see their own mistakes and can't see any of the good their teammates do. Watching your replays will help you correct that.
