Should I stay or should I go?
I've got a really bad habit of either backing too often or staying too long in lane. When should I back and when should I stay? Are there any situational factors to take into account?

Depends when, If it's your 1st back then the biggest thing is gold. Do you have enough to get a "good" item. But thats only when you and the other top dont really trade. After a big trade the loser of the trade generally backs, and the winner shortly after finishing the cs. After that its basically whenever you feel it is right to leave. Having a good map awareness is key. Knowing where the enemy is key.

Most adc champs get a major damage item early, and a key item for that is the BF Sword
, which costs about 1550 gold.
Most times, when you first back, you want this item. If you manage to acquire this amount of gold, back for the item, and return to lane (push your lane first. maybe get a kill) That might not happen however; strong lane opponents can have you in critical condition before then. If you're at less than 25% hp, back.
Now, in terms of objectives, you normally want to back after acquiring an objective. Turrets especially, but sometimes you can do so after dragon, red/blue buff, or baron.
