Gold mid/supp main looking for a team

I am a Mid/Supp main looking for a team that will be dedicated, striving to grow, and ready to consistently play through the season. My main champs are as follows: Mid - Nidalee, Twisted Fate, Azir, Orianna, Yasuo, Vel'Koz, Kennen, Jayce Supp - Blitz, Bard, Karma, Thresh, Janna, Leona (My absolute best being Nidalee and Blitz) I am very good at communicating with team, paying mind to vision, and team fighting. If you would like to know my weak points, I would say that my early game (lane phase) is not as strong as my late game (ability to team fight), and I am not the best when it comes to CS. However, I do usually win my lane, and if I don't, I always make up for it in the late game. I have all the voice chat options available, and am available usually any time on the weekends, and after 6pm on weekdays. Personal info: I am 21 years old, in college, on EST time.
