Making a Gold-Plat team

Hey everyone...I've recently joined a few teams over the last 2 months only to find them disband after one or two losses...guess what we are going to lose every now and again...its what we take from the loss that matters...I'm trying to make a team that will stay together and discuss problems w/o anyone getting butt hurt...I am a Mid/support main...so i'm looking for all roles...also our play time will be around 7pm-11pm eastern...3-4 times a week...make sure you can fit this time into your game time...please fill out this short app as it will help me organize a tryout...Thanks guys/girls

Additional information: I have great communication, I am a team player and will do whatever the team needs. I dont mind dying in order to peel for a teammate or to set up a team fight. And, like i said, I will do whatever it is so that the team can win, and to improve my skill. I do not rage and am very calm, and when behind, I always believe that we can come back and dont give up. I take criticism very well and actually encourage it and will admit my wrongs.
