Creating Ranked 5s Gold and Up

Greetings,

So I'm guessing that many of you, like myself, have been searching for a ranked team. All we find are teams that never do more than their 5 placements and then if falls apart. Well, I would like to make a team that actually attempts to climb in ranked 5s. I am a Gold 4(currently) mid lane main and am looking for the rest of the roles to be filled. I'll put a simple app on here to fill out but add me as well. StockSyndrome

Biggest Strength: Playstyle, game sense, movement, timing, engage and disengage. when to trade, lots of 5's Experience, played in 2 high ranking teams with diamond 1 adc's

Biggest weakness: Hard to concentrate and communicate at the same while in high levels of play especially big teamfights. When in lane or gank, and I see a beneficial opportunity in our favor, I'll take it, usually in half a second so little time to say,"go in" "going in" etc just hard to concentrate and communicate at the same time. Could be their misstep or when they have obvious CD's on abilities Ill take the opportunity aggressively. Also, I get frustrating sometimes when the adc & I aren't in-sync and cant play off each other's strengths/ make up for eachother's weaknesses.

Top 5 Champions: Ezreal Vayne Zed Thresh Lucian

Strengths: - Have experience of being in a ranked 5s team. Played with diamonds/plats

Have experience of being in Diamond.
Know the difference between playing in Solo queue and Ranked 5s.
As an adc, exceptional positioning, mechanics, and game sense.
As a support, know how to play lane with different match ups, mechanically sound, and prioritizes the ADC (as all supports should!!!)
Adaptability, I can play with different supports and adapt my play style to how they play.
Not toxic at all and I can take in criticism as well as give, if I have the right to do so.

Weakness:

Map awareness may fall off sometimes, especially when I'm tired. Will need teammate's help, which is why I like having a support!
Mechanics are still not 100%, as I am coming back from taking a break for a whole season.
More of a listener, rather than a vocal person.
May tunnel vision sometimes when I get too greedy. Will need teammates to ping me back at times.
