Get lolreplay, start watching yourself play. Be objective, and do side-by-side comparisons to what you do vs what [good players] do. Every time you die, look around the area to see what caused you to die. Assume it is your fault, that you made some miscalculation, and think what you could have done better. Do this with missed kills, failed objectives, etc. Every time you DO get a kill or objective, look at the game and see what the next move should have been, and how your moves were different.
Assume that every death on your entire team is the result of a mistake you made, unless you can reliably prove otherwise. Would watching the minimap and pinging the enemy jungler you spot near dragon have saved bot from a deadly gank? Then it's your fault that you didn't ping it. If you did ping it, you could have pinged more fervently. If you pinged more fervently, maybe you needed to ping faster, etc. Only if you can 100% tell you didn't do anything that caused that death, should you accept it wasn't in part your fault.
Start studying. Learn and memorize as much as you can about as many things as you can. Do you know when creeps spawn? Jungle camps? how long do they take to respawn? how about dragon? Baron? Do you know the base CDs for all the typical champions in your preferred lane? Do you know how often siege minions spawn? There are a thousand things you can learn that will help you calculate your chances of doing something useful at every moment of the game.
Probably easier yet just as important: PRESS TAB. Know who has money, and how much. Don't be caught surprised by the enemy buying a hexdrinker and suddenly killing you because you failed to account for the shield proc. Learn to adapt to the flow of the game. Sometimes you don't actually want to rush a big item, you want 2 midlevel items because there's a lot of brawls happening and you need that extra edge NOW, not in 5 minutes when you finally complete that item.
Sorry for the very matter-of-fact tone, I just wanted to list as many things as I could in a short time, and didn't want to waste time on eloquence.
