Season 4 Diamond 5 Top/Jungle main l4 team

I was Gold in season 3 and diamond 5 in season 4. I usually play whatever role is open, but I have been inactive for a while but am trying to get back into the game. Would like to find a team that plays often. That has a leader that gives constructive criticism. I mostly jungle now instead of top. I know i'm a little bit rusty but with a bit of help i can get back to performing very well in the jungle and/or top. Don't bother adding me if you are going to rage for little mistakes i make, but instead communicate and help me fix those mistakes.(also i'm a bit shy so i don't usually talk often. but i do listen very well) IGN: Broken Chairs
