Looking for Subs for Ranked 5s [Silver 2+ ONLY}

Counter Curse Mid is looking to add subs to our roster. We typically play between 8pm and 2am PST and are an objective oriented team. If interested please reply with completed form including top 5 champions for primary and secondary positions. We are looking for subs for every position but most in need is top lane. Top lane applicants will be a short interview while other applicants will be through interview and try outs.
