This is an extremely difficult question to answer. With you being Plat rank, I would assume that you have great knowledge of the game and thus understand what it takes to win the game.

Back when I was Gold 5 I ran into a similar problem.

I would win a game lose a game. For several HUNDRED games I would not climb out of Gold 5. I was extremely livid because I thought that I was good enough to be Plat (which I am now). But it wasn't until I sat down and looked at why I was losing games.

I got to Gold 5 playing nothing but mid lane. Once I got to Gold started playing a lot of support Thresh. When I wanted to win again, I started thinking to myself, "Maybe I should go back to mid lane." That's when I found immediate success, I climbed that ladder fast, in a matter of days I was Gold 2, playing 2-3 games a day. But then I started having more stale games, lose one win one lose one win one, so on and so forth.

Then I started looking at everything else I didn't know, I knew I could not play Top or Jungle with my life, if I went to either of those lanes I would feed super hard. So I knew that if I improved my game in those roles, especially in Jungle, I would get better because I would be able to keep track of the enemy jungler and timers, protecting me from ganks and maybe getting a cheese off on their jungler at their blue. I was also able to find out about this new cheese that was rising whereas, the jungler, you would get a smiteless leash at your Gromp and walk down to the enemy red, smite steal and maybe get a kill. This allowed me to place wards for my jungler (to protect them from this), if I ever went mid or bot.

Now, I'm telling you this because climbing the ranks in upper ELO is based off team play. Me warding to protect my jungler from the lvl 2 cheese didn't impact me directly, but it sure did help me win many games; I guarantee that my jungler would spam ping help, end up dying and rage quit.

In Bronze, Silver and Gold; there is very RARE teamplay. 80% of the time you have to carry the game yourself and get to a division where you don't have to carry anymore; rather, you need to work as a team. This is why Katarina isn't that big of a deal in upper ELOs, people know how to and will shut her down. So you need to focus on what your team needs and what the enemy team doesn't want to fight.

I understand this was extremely vague, but with you being higher ELO, there isn't much I can really teach you.
