Long story short: I have performance anxiety when it comes to playing PvP....(It's pathetic I know). Looking for a group that could help me get over this fear and must have patience to put up with my negative attitude.

I am getting nervous just by writing this....

First thing you need to do is get rid of the negative attitude. Always be positive when you play. It's not the world if you have a bad game or lose. Just keep practicing and you will get better.

Always carrying a bad attitude when I die....I feel like I am feeding and I don't want to let my team down.

If you have the wrong attitude, you will make yourself go on tilt. If you think you are feeding, you will feed.

You just have to realize that EVERYONE had to start somewhere and they sucked and fed and lost. You just keep playing with determination and you will get better.

...And That's why I stopped playing PvP and went back to playing bots.....

Bots is a great place to learn the mechanics of the champions and how to CS properly. If you get those down, you will have success.

Get on a search engine and look up guides to the game. There are a TON of great ones out there to help you improve.

I don't want to drag out the conversation too long, but the reason I wrote this is because I was shivering every time I quene up for normal games. Now, I didn't feel like this back then, PvP was a natural thing for me but now....My knees were shaking, hands went cold and felt this immense pressure to do well so that my teammates won't have a bad game and do really really well. And the result? Oh, boy it was not pretty.....I started to go tilt every single game I played, felt extremely bad if I gave away first blood or died too much, and the negative things in my head wouldn't stop...To you, it may seem like I am over-exaggerating, but it's the honest truth. (I also dread sharing personal stories like this). I jumped into a PvP quene once and my brain was sending me a message like: "Get out of there or you'll die and risk losing again."

So, I panicked and left the quene. So, I asked various people on the fourms that playing too many bot matches were okay (Even though the Co-OP Vs. Ai is kinda frowned upon in the community.) And they answered that I should enjoy the game however. But, in the back of my head it felt like I was being left out...Sure, Co-Op helped me to get comfortable with the game but I get always intimidated when my friends asks me over to play PvP with them and would always turn down their offer.

I guess I developed a perfectionist syndrome too, seeing how I don't want to lose and all....Great. (I am such a loser).

It sounds like you have an anxiety disorder. You seriously should get checked out. I have GAD (General Anxiety Disorder) so I know how crappy it can get.

Hate to admit it, but I think so too.
