Creating Ranked 5's team

So with the season coming to a close very rapidly I believe it is time for me to reopen my ranked team from last year. Almost all of the players on the team quit playing league so creating a brand new roster for end of season rewards and possible future competitive play. All ranks are welcome to apply but please fill out the application here. This will help me organize applicants much faster so that I can get the right players for the team. Last season the team finished in gold 4. Below is a list of things I am interested in, please take the time to fill it out. If you want to add me after completing it that's fine, otherwise ill add you. If you just add me in game though and do not fill out the application below you WILL NOT BE CONSIDERED and your friend request will simply be ignored.

Flexible, only lane I have not soloed to gold this season is top but can compete in the other 4 roles. Schedule: I work full time so only available after 5:00 cst and weekends. Don't have a lot planned currently next couple months so could do some ranked 5s grinding. Could maybe sub if team looking to be more hardcore and play when I am not available.
