LF Top/Mid/Jung/Support Serious Ranked 5's

Hello there everyone! I am putting together a new serious team and am looking for all roles! (except adc) The team will have practices 4+ nights/week and will expect very close to perfect attendance. I am going to keep this intro short, but will include requirements, as well as a template to fill out.

As of right now, our tentative practice times will be mon-thurs 6pm+ PST. Obviously these can change depending on the members schedules, but this will be pretty close to practice times/days.
