LF Team to play on for Season 5

I mainly play as an ADC but can fill any role except top... and like in the title i am looking for a team for season 5... i am currently Plat 4 but was Plat 2 earlier in the season... just kinda fell off due to lack of care.. but for season 5 i am looking to play on a ranked 5's team and hit Diamond while doing it... so please if you are interested in me and if you want me on the team..
