Ranked 5s team looking for Gold+ Mid, Jungle, and Support

Hello, me and a friend are wanting to make a semi-serious ranked 5s team. The main goal for the team is to get better as a team, and individually as well. Fill out the app I post below and add me on league, IGN is personwithface. I may not be able to get to everyone tho, especially if I find someone who I think would be a good fit for the team before you. Like I said in the title, looking for mid jungle and support, if you're not one of those 3 I will not be adding you.
