Making a serious team to make it somewhere!

Hello summoners! I was a former platinum 2 player a year ago played on a all diamond team, but unfortunately I moved and lost my rating due to inactivity. Now I'm back I'm looking to make a change in the game, to become one of the best. I am looking for 4 other skilled members willing to take it to the next step in the path of becoming pro. When I say serious I mean it as well, we will be practicing at least 4 days of the week. If we can train 5-6 days that would be lovely. But I do understand people have lives. If you do not show up for a scheduled practice without a warning you will be benched. If you can not communicate or meet my requirements please save yourself some time and apply elsewhere, this is a team game! Now for the application/requirements.

Requirements!

1. You will fill out an application that way I know if you're actually reading this or not. Anybody who adds me will be ignored/refused this way I can keep everything in order and see if you can follow basic directions.
2. You must at least be a gold+ player NO EXCEPTIONS!
3. No negatively, if somebody makes a goofy mistake wait till AFTER the match to bring up what kind of improvement should be made. Once we're in game we need to be focused.
4. Understand the basics ex. Warding, Lane pressure how to push a lane, how to slow push a lane, how to freeze a lane etc.
5. MAIN YOUR ROLE!
6. Communication with no back ground noise, we will be using curse voice.
7. Be able to give and receive criticism.
8. 0 Excuses, we are looking to be the best so you mental game must be strong!

My application: Superdurp Current s2 on this account, g5 on smurf Platinum 2 in solo que, Diamond 3 in ranked 5's Main ADC, back up is top ADC pool: Anybody still working on kalista Former experience: I've been on many ranked team's as well online tournament teams. I have only ran 2 teams mainly played as a player. Goals are to become one of the best players for my lane and to eventually make the LCS. Those are long term, short term is finding a dedicated team that's willing to keep improving to become flawless.

Extra info: This is a mental game, be prepared friends.

Will take coaches who want to help out but as of right now we're in the building stage.
