Hello summoners,

Im a Gold league Jungler that has been wanting to create a 5v5 team that stays together for quite a while now. I've had experience in the College ladder and have been part of various tournaments. Here is the list of requirements some being hard caps (for various reasons)

Age must be 16+ (hard cap) Rank must be at least Silver (hard cap) Must be mature enough and dedicated enough to come to scheduled practices at least 3 times a week (HARD CAP) Must have voice comms (hard cap)- Pretty open to which we use and will be specified later

With that being said we will need players of all postions. Please copy this form and fill out the specified areas

IGN: ZanTwilight Age: 18 Role: Top / Mid Voice comms (which program you use): Skype, Mumble, Teamspeak, willing to download whatever 3 best champions: Swain, Nautilus, Poppy / Swain, Lux, Veigar Strengths: Been playing for a long time, so I have a good understanding of macro and am at least proficient with most meta champs. Good at analyzing games, can give and take criticism. Weaknesses: Can sometimes tunnel (generally better when in a voicechat), trouble playing from behind Reasons why you want to join a 5v5 team: I love game analysis and strategy/shotcalling, which isn't very useful when 4/5 of your team is ignoring you. I feel like growth is much more steady when in a 5's team, and games are much cleaner due to better trust and communication.

IGN: NurfedYou Age: 25 Role: ADC Voice comms (which program you use): Any, I have most already downloaded 3 best champions: Twitch, Ezreal, Lucian Strengths: Teamfighting and Skirmishing Weaknesses: Playing losing matchups in lane Reasons why you want to join a 5v5 team: I've played 5's on and off for 2 seasons. Makes the game actually fun and you have 4 other people to help you improve your gameplay

IGN: EvanPenning Age: 23 Role: Top Voice comms (which program you use): Skype installed, but open to any 3 best champions: Wukong, Volibear, Warwick (Don't laugh, it's actually pretty good) Strengths: Split pushing, playing a high mobility game, creativity, playing bruisers and tanks, friendly player, and willing and wanting to learn to benefit the team Weaknesses: Playing as ranged champs, playing as slow squishies, and sometimes tries too hard Reasons why you want to join a 5v5 team: I've played on two teams in the past that for various reasons broke up. I love the team environment and having consistent people to play with. I like knowing my teammates' play styles and them knowing mine. I thoroughly enjoy working on strategies and team compositions based around all our strengths. I love competing (former college athlete) and working on improving skills.

I've been a Silver player the past two seasons. Currently Bronze II, and working on getting back up in the ranks

IGN: KiIIz Age: 18 Role: ADC Voice comms (which program you use): Mainly Skype but can also use other communicating sources. 3 best champions: Jinx, Caitlyn, Lucian Strengths: CSing, Great mechanics and Teamfighting Weaknesses: Sometimes being too aggressive during laning phase Reasons why you want to join a 5v5 team: Improve my play and to compete alongside other competitive players as well as acquire new skills from them. Overall to win in general.

IGN: Innocent1one Age: 21 Role: mid Voice comms (which program you use): Any, will get any I have never heard of 3 best champions: Cho'Gath, Anivia, Annie Strengths: I am strong mechanically and have a good general knowledge of the game. Weaknesses: I can be overaggressive when trying to benefit the team leading me to be in less than safe positions. Reasons why you want to join a 5v5 team: I want to join a team so that I can have some form of cohesion and a common goal. I want to work with people to improve together.
