For the sake of my sanity I'm going to assume you're asking about lore in the modern era and not ancient history or the like.
So right now Runeterra's got a bunch of stuff going on with it. Ancient powers are stirring, calamity's on the horizon and World Runes are reappearing, spurring in to action beings like Bard and Ryze, who're intently focused on their own ideas about preservation. But while these guys work to keep everything from going to shit, the world's moving forward and practically tripping over itself with the amount of dangerous stuff appearing.
On the Shadow Isles the Black Mist runs rampant and we have powerful entities within it trying to manipulate the mist to its own ends. Mordekaiser wants to build his empire of undeath up once again, Karthus wants to have everyone be enveloped in the mist because of his ideology involving it, Yorick wants to destroy the mist from within, and Kalista just wants to kill betrayers. These beings aside from Kalista vie for power on the Isles and are all working to bolster their own forces with the souls of the dead. Maokai, a spirit twisted by the mist, is also looking to destroy it and restore the Islands to their former glory, but in his own way.
Mordekaiser's being most proactive about raising his armies by manifesting the Mists in the form of Harrowings that he's been sending out to the Serpent Isles a lot. He's killed a lot of people doing so and only recently has Bilgewater mustered the might to repel his forces in a meaningful way, in the form of the power of Nagakabouros; the principle deity of the Buhrun people.
Speaking of the Serpent Isles Bilgewater's had a busy time recently, as Miss Fortune finally went and enacted a plan to kill Gangplank, and almost succeeded, but this didn't have the effect she had hoped for. Gangplank as the Reaver King kept all of the gangs, pirates and shady folks in Bilgewater in check, and without him the city's descended in to chaos. Gangplank, while still alive is believed to be dead by everybody, and is looking to build up his power once again and reclaim his title and power. Miss Fortune on the other hand wants to fix Bilgewater for good, and is working towards beating down the rampant crime and pushing Bilgewater to a brighter future. Illaoi, the Truthbearer of Nagakabouros and the one who saved Bilgewater from the most recent Harrowing, looks to help both Miss Fortune and Gangplank in their goals, as either one can help fix Bilgewater in her eyes, and it's not her job to choose who, only to help the process along.
Across the sea on the southern continent Shurima's been a rumbling with activity. Because of the actions of Sivir and Cassiopeia huge changes have occured in the barren deserts as Azir, former emperor of Shurima, was resurrected and then Ascended, using his power to raise the city of Shurima from the sands once again, while his betrayer Xerath and Renekton, the Ascended warrior who has been driven mad by Xerath's machinations, were released on to the world. While Azir works to restore Shurima by terraforming the land, restoring the ancient waterways and rebuilding the fallen empire Xerath seeks to make his own, but a world encompassing empire where all will follow him. Nasus, Renekton's brother, seeks to not only save his deranged sibling but also stop Xerath's plots, and recently had a confrontation with the Magus that didn't end too well for the old dog. Shurima's future is uncertain, and its people are caught in the middle as ancient powers are being unearthed from the sands.
From the depths of Icathia the Void seems to bubbling out as well, with beings like Vel'Koz and Kha'Zix emerging in Runeterra for the first time. The Void remains a mystery, but Runeterra has assistance from beyond the stars in the form of Targon.
The immense space faring empire of Targon is having problems of its own though, as inner conflict stifles their work. Aurelion Sol, long enslaved by their magics, has returned to Runeterra after eons with the hope that this tiny world holds the key to his salvation and Targon's undoing. Meanwhile on Mount Targon the Aspects from that space empire have begun selecting warriors to fight for them in numbers unheard of, with currently four humans being chosen as vessels for the Aspect's ethereal powers. Pantheon has taken complete control over a Rakkoran warrior named Atreus, while the Aspects of the Sun and Silver Moon have chosen Leona and Diana respectively as hosts, while The Protector has chosen the exiled Demacian Taric to be its vessel. All of them fight for their own reasons, and Targon looks to steer them towards some ultimate goal, with Diana and Taric combating the Void in earnest, Leona relentlessly pursuing Diana, and Pantheon preparing to face Aurelion Sol.
Of the two nations that dominant Valoran, Demacia has its hands in its own business almost exclusively. What they're up to as a nation isn't know, but their soldiers fight tirelessly in the pursuit of justice as always. On the other hand Noxus has its hands full considering that they've been involved in a massive scale invasion of Ionia which either only resolved recently or hasn't resolved at all yet. Swain leads them as best he can and a glorious future seems to be looming for the empire with him at the helm.
In the Freljord tensions have never been higher as the Avarosan and Winnter's Claw continue to engage in brutal warfare against one another. Lissandra and her Frostguard still plot to bring about the return of the Frozen Watchers while folks like Trundle, Tryndamere and Volibear have struck up alliances to secure the futures of their people, but right now there's no clear victor in sight.
In Ionia there's a lot of governmental problems as the nation reels from the extent of the Noxian invasion. New forces within the governing body of Ionia push for aggressive advancement of magic and technologies, leading to the creation of Ionian hextech, and many people still struggle with the realities that came to be as a result of the long war. Massive changes are a foot for them as they struggle with the needs of tradition versus their advancement as a nation.
Piltover and Zaun are kinda just doing their own thing right now, as far as we know.
Yordles are weird and remain weird in their strange unpercievable city hidden by magic.
That's about as condensed as I can go with it right now, but I can give more specifics if you so desire.
Pretty much just a generic fantasy world mishmash. Evil baddies everywhere and people die 24/7 now because it's edgy, and they're yet to tie all the loose ends. With how things like Maokai turned out from a lore perspective, my expectations aren't that high :P
Its pretty much stale atm, no factions are directly engaged with one another in open warfare. We get the odd Terror for Bilgewater annually, But nothing cataclysmic is going on, Less Malzahar is cracking open Icathia and the Mount Targon aspects are preparing.
