I'm pretty sure that this happend cuz of the FOV from the camera. Perhaps its an effect added. Objects that are closer to your eye appear to be bigger. It has nothing to do with her hand being enormous.
They are most likely this big because you'll see Akali from a top-down-view in the game and it'd be hard to see her hand-movements with the camera zoomed out so much.
