Guys, I figured it out and it makes so much sense. Okay so first let me explain quickly that the basis of all this working surrounds the idea that 'A New Dawn' happened after the Lore retcon. Comprende? Okay, let's continue.

Summoners Rift is a place in Runeterra one does not go to, they are brought there, and if brought there you will remain trapped there until you fight your way out. To do that, the champions must destroy the enemy nexus in order to return to their lives as per usual until they are summoned again. How they are summoned, who really knows; it could even be a place champions are forced to visit through their dreams. I know it sounds crazy and you're right but right now I really want lore connected to the game again. Before you leave though (thanks for staying this long actually) let me point out a few things that support this.

First off is the one major one I figured out; when Nautilus enters at 3:20 ish. Watch how Darius reacts. Go on, watch and I'll wait. You back? Okay, did you see him? He wasn't expecting Nautilus and didn't even really work with him. Nautilus is a hulking titan that so happens to be on Darius' team this time, and when you've got something like that to work with you just throw stuff at it for it to destroy. Nautilus wants to leave the rift too but he could care less how he works with these people he doesn't even know, he just knows he needs to work with them and fight the ones against him (Graves). One other thing; Naut was using an elixir of iron.

There are a few other things in the video you can link to this theory after you notice the big one, such as how Leona and Ahri are wandering about trying to figure out where everyone is, how Zyra carelessly lets Draven try to kill Ahri since he can carry harder with his AD (and she was taking the role of support), then at the end how Rengar, Ahri, and Leona are looking at their goal to freedom right at the end. They want to escape the rift. This idea sounds kinda half baked in writing since the thoughts came to me late at night and I was not entirely in my right mind but it made so much sense I had to share.

It's more a hopeful dream, and for now my way of tying the Lore back into the game more than anything but it makes sense when you connect the dots. Let me know what you think and please, speculate away with me :). If you read this whole thing I just wana say thanks for not just brushing this idea aside :P. Go, converse, bring up points on what makes sense or what doesn't :).
