Worst: 
She is an officer of the law, from a high class family, in a major city with high standards & expectations of it's citizens. In no universe does her outfit fit that thematic.
Great visual presentation. She looks classy, yet badass. I know, people hate the whole blades for legs thing, but I always liked it. It gave her something menacing, while still feeling elegant.
This guy is the worst for me, hands down. Everything about his ingame art makes a joke out of him. He looks goofy. His quotes (the few he has) are just some bad jokes and a ridiculous laughter. This guy is supposed to be a cruel scientist, lacking any humanity. He is intelligent, methodical, scary. I wish they would give him the Ezreal treatment, so I can truly see the guy in the game from the story "Engineering the nightmare".
I'm gonna pass on the older champs, because they were made for clickbaiting rather than theme-matching visuals (otherwise, I'd say LeBlanc. Her visual is more like Saturday night strip-tease than deceiver, eh). So without further ado....
This one I think I don't need to explain. Even Riot admitted their mistake with her. She looks more like a model than a void-survivor, and even though she was not in the actual Void, that still doesn't excuse her design.
Now, hear me out. Syndra's my absolute favorite champ, and I love her design. But it's crap. She looks like BDSM-Queen, not like Ionian dark powerhouse. I understand that she has to stray away from Ionia's traditionnal pattern, but her outfit... No. Almost everything is fine about her, but not her outfit. The colors are good, just... More clothes please :')!
Not much of a fan of this champion, not gonna lie. I don't like his gameplay, and lore-wise I'm team LeBlanc. But his design is really cool, and fits his themes, not much to say about that.
Aaaand Evelynn. Ah, Evelynn. I think her visual design really fits her theme. And I love her quotes. She's my favourite VGU so far (well, not for long since Morgana is coming but shhh). And I'm not saying that because "Jaja she's a hot thot". No, her design is just perfect for the malevolent demon she is.
Ornn is the best. He really upholds to everything you expect from him. A grumpy, blacksmithing god. He loves drinking, fighting and eating. He is also sorrowful because of what happened to his followers. I just want to hug him and he knows it.
I have fur. It is soft. Do not hug me.
For worst, Malphite. He needs a rework hard.

is definitely the worst in terms of kit matching visuals. A hulking metal man with a mace the size of ten teemos is... a squishy AP burst skirmisher.

I think at the moment, the worse are the ones said in this thread. For different reasons.
Caitlyn is a relic of the past, she is this reminder of the cartoon way that Riot was making character and how the lore couldn't live ever to their thematic.
Singed is honestly such an incredibly "evil" character in the lore, even "despised" by the noxians. He has SO SO SO much influence on some characters and yet, Singed is cartoony and weird. But not the good kind of weird, he is just flashy in colour, all over the place, he is an OTP pick that doesn't fit its thematic.
And... yeah Malphite. Model is horrible, he is like... He is a titan, even like a chaotic force in some sort, a force of nature... And like... Gosh. This poor boy.
I think like MANY of the new champion have amazing design that match their characters. Same for the VGU. Mention to Ornn & Urgot.
i think akali has the best visaul to charecter . she a rouge ninja and really stylish with her killings.
I think Kha'Zix is the coolest looking champ, love his voice too. I don't pay attention to lore really.
Easily the best. They really captured the "werewolf" and most importantly, the canine feel of Warwick. Watch a Schutzhund video and look at how the K9 will chase, dive, and latch onto an assailant arm even as he spins him about. Then watch a clip of a Warwick with his W activated, running into a lane and using his R and then Q'ing through them. It's something a lot of people don't get but enjoy seeing (so long as it's on your team). That attention to detail was not lost on me with the rework, and really makes Warwick feel like an actual canine instead of a human with canine features.. 
Worst thematic because it's like.. wtf is Mundo? I don't even know. He throws an infected cleaver. He has a spinning belt of fire. He does some drugs and heals up massively. I want to like this champion, I really do, but I can't.
