Question on Kalista's lore/wording error?

"She rammed the spear into her chest, driving it in deep."

(sword-wife) rammed the spear into (sword-wife's) chest, or (Kalista) rammed the spear into (sword-wife's) chest? or, troll answer- (sword-wife) rammed the spear into (Kalista's) chest?

Bothers me a lot the way that was worded. Does the person have to forcibly, physically take their own life, or does Kalista do that part for them? Changes the context a bit.

I'm leaning more toward Kalista doing it, because I don't see the sword-wife having the strength (physically, or of will) to drive the spear all the way in.

Considering that the previous five instances of "she" have been referring to the sword-wife, and the lines preceding and following that one are about her, it seems unlikely to me that Kalista was the one responsible. It would make no real sense to use "she" to refer to Kalista there rather than explicitly naming her.
