Is it me or the runeterra map is old and very small and does not have much geographical since what do you guys think.

The old map isn't canon anymore. Shurima for example is now on a different continent(and Ionia is also its own continent as well).
the map is outdated since like month ago, ichita has been move east to west and shadow isle did the reverse. Riot is making a new map be patient.
The map you're referring to is outdated and not canon anymore. The Narrative team is working on a new map and cool, new ways to deliver it to players.
