This literally just bases off the fact he has a hook. He is an inhuman champion with inhuman features and going off that there isn't much they could do to make him look like your average joe fisherman or even something closer to the vibe of Pyke or someone from Bilgewater. He is to far from human and they certainly won't go so far as to remove his signature floating head tendrils and make a generic human look for him just for a fisherman skin.
