New champ idea "Zalazar" Support/jungle, Could it work?

So I had this idea about a possible new champion, and this was the only place it made sense to write it down, and get a feedback or review. I wanna talk about how he looks, I have drawn a picture of him already, but I will describe him instead. His body is like a salamander, 3 times bigger than a human body. But the head of the salamander body is removed, and an upper body of a human is shown instead of the head. He is muscular and has a fade haircut, and "elf ears". (it's like the human horses from Narnia, just a salamander body instead of a horse). On his back as a salamander, he has spikes. He can transform into a human but still has his tail and spikes. And he is an AD support/jungle champion. And should be played tankie, it is possible to play him as a counter, but he is primarily a tank champion and a healer.

Abilities : Remember all the "(+0)" is physical damage stacks.

P= "Magic hands" His passive is an auto attack that hits physical damage. He uses his hands to shoot out "salamander spit" For every 5 auto attack hits on an enemy champ with 1,5 seconds in between every auto attack, it will stack +0,7 physical damage. For every 5 auto attack hits on minions with 1,5 seconds in between every auto attack, it will stack +0,2 physical damage.

Q= "Spiky Spikes" He shoots spikes to stun his opponent for 1,7 seconds. He has a combo of throwing two spikes, with 0,3 seconds cooldown in between. It will damage 90 (+0) physical damage. Cooldown: 15 sec. Mana cost: 45.

W= "Lost Tail" He loses his tail on the ground to pick it up again to regen 240 HP + his own currents HP. Tail last for 20 sec on the ground. Cooldown: 10 sec. Mana cost: none.

E= "Strong heal" Heal allies in the range of 20 with 160 + his own current hp. Cooldown: 20 sec. Mana cost: 60.

R="I'm a human" Zalazar transforms into a human and earns 65%(+0) attackspeed and 50% movement speed (+0) and +660 (+0) HP, for 15 sec. Cooldown: 60 sec. Mana cost: 100.
