[Champion Concept] Inis, the Lost Portaler

So this is a champion concept that popped into my mind in the past days, I'm not so sure about how balanced he is, but numbers are mostly just representative. Let me know what you think and feedback is welcomed! I tried to come up with a little lore concept too, so to give the character a feel as of what the character is trying to be. I'm not that great with English either, so my apologies for that.
#Inis, the Lost Portaler
#Short concept: Inis is supposed to be a mage character who is CC oriented while still capable of dealing fair amount of damage and with a combo he is also capable to burst down one target. The character is mostly themed around portals, and teleportation.
#Lore concept:
Inis is an artificial being accidentally made by an experiment. He was an outcome of experiments by a group of mage in Zaun who were trying to create temporary and portals. These mages united their powers and magic to create something new, a portal that would grant access to different places of Zaun by an instant when entering the portal. When the group created the first portal, it seemed to be stable, but it didn't last long. As soon one of them tested the portal, which indeed was successful, as soon that mage used the portal it gone unstable and energy blasted out of it, which hit one of their empty garb hanging on the wall. The hollow clothing began to move and soon it formed a visible human-like shape made out of magical energy. The group of mage cautiously observed the freshly appeared being, but it didn't seemed to be hostile, in fact the being acted friendly toward the mages.
They granted him the name, Inis and he quickly began to communicate normally. The mages in an exchange to examine and study him, they created him an artificial head which allows him to perfectly see all around himself. The mages soon noticed, that Inis seems to suffer from a glitch effect, that makes him and his power temporarily unstable. Inis seemed to be rather human-like, he was aware of himself and his abilities, but he also seemed to have certain thoughts and memories appearing during his instabilities that he cannot understand or clearly remember. He thinks and feels, that he has existed before appearing in Zaun.
Using his abilities, he now travels alone around Runeterra seeking out for powerful mages or creatures for possible help, along those, he tries visits every place and person on Runeterra, he wishes to find something or someone, that would make him remember something clearly for him.
#Appearance:
Similar to the picture below, except that instead of having a fleshy creature, he is made out of vibrating energy that forms a human body-like shape out of the garb while the garb itself is hollow inside. The head of Inis is mostly a special artificial head that is supposed to be rather similar that is seen on the picture, with a sphere inside the middle to allow him to see everything around himself.

#Roles: Mage - I think he could fit both for mid and as support too, or even maybe top too?
#Abilities:
Passive - Portaler Instability:
Every 21 seconds Inis enters an instability state, that lasts for 4 seconds. When he enters his unstable state he begins to create a randomly placed portal on the map that turns functional as this effect ends. During his instability has no cast time (can move freely without movement interruption from casting abilities) and his next ability will deal 7% (+1%/level) increased damage.
Portals lasts up to 70 seconds and they are connected linearly by time to each other (each portal is connected to the one that appear after it) and cannot appear inside the enemy's base or near enemy turrets. Inis is able to interact with his portals to travel through them (0,75 second channel time), but any damage taken will interrupt him, if he is successful, he consumes the entrance portal. Enemy Champions can interact with the portals to destroy it, if an enemy Champion destroys one of Inis' portals that champion will be disoriented for 1,5 seconds.
Portals cannot be used in combat.
Q - Shifting Energy:
Inis sends out a ball of shifting energy in a line at a targeted location. If the orb hits an enemy Champion it deals 80/120/160/200/240 (+45% AP) magic damage and displaces (teleports) the champion directly away from Inis' current location. If the orb hits a terrain instead, it creates a temporary portal through that terrain, that lasts for 2,5 seconds, Inis can interact with and use this portal once to teleport to the end of it and its mana cost is refunded. Shifting Energy can pass through minions and monsters, but deals 25% reduced damage to them.
Displacing someone through Temporal Rift will cause it to crack once.
Cooldown: 10 seconds Mana cost: 70/75/80/85/90 mana Cast range: 900 Displacement distance: 550
W - Temporal Rift:
Inis places a wall of Temporal Rift at the target location that lasts for 5 seconds. Each time a friendly or enemy unit passes through the Rift they cause it to crack, additionally enemy Champions passing through the Rift are slowed by 15%/20%/25%/30%/35% that decays over 1,5 seconds. If the Rift is cracked 5 times, it collapses, dealing 70/100/130/175/210 (+55% AP) magic damage near its location and to enemies who passed through the Rift. If an enemy Champion passed through the Rift more than once, the damage is increased by 50%.

E - Unstable Portal:
Inis creates an Unstable Portal on the ground at the target location, that quickly expands over 2 seconds. Enemy Champions that touch the surface of the portal each time (maximum twice) will be disoriented for 0,75 second, causing their desired movements to go to the opposite directions. Minions and monsters are feared instead. When the portal collapses it deals 75/110/145/180/215 (+45% AP) magic damage to any enemies within and near its radius.

R - Separation:
Inis marks an enemy Champion for 1,5 Seconds and creates a temporary portal under its feet. If the marked enemy Champion can't escape from it's range, the portal sucks in its marked target, dealing 100/150/200 (+25% AP) magic damage and displaces the target temporarily for 4 seconds to a random place on the map (cannot teleport enemy to own base or allied turrets' range). When the effect ends it returns the target to the portal's location and deals an additional 100/200/300 (+45% AP) magic damage to the target upon arrival.
Cooldown: 190/150/110 seconds Mana cost: 150 mana Cast range: 850
#Ideas for quotes/personality:
I can't really come up with great quotes myself, however he would talk about his journey, that he wants to see different places all over Runeterra, different person and "champions", he is also kind of interested in void creatures, he basically wish to see someone or something, that could "trigger" any of his memories and make him either remember or help him some way. He simply just want to see everything, so maybe something he sees is very similar to what he sees during his instability state. During his instability he would probably talk in a different, deeper and echoing voice tone and incomprehensibly as if he would just mutter something to himself that nobody can understand at all, since that is the time he gets these sudden memory flashes. He feels quite lost as of who he actually is or was.
#Death animation:
I thought of this, and to me he is kind of an "immortal" being, and instead of dying, he simply uses a portal to get out of the scene after he is defeated and his respawn animation is basically him walking out of the portal.

Update #1 removed slow from Temporal Rift (W) and added a note to Q that it can crack Temporal Rift (W) if it displaces a champion through it.
