I have seen a lot of 'a player flagged by your report' popups. At LEAST one a day, sometimes two. This directly followed me changing HOW I report. It's down to three simple rules:

1: Avoid "Unsportsmanlike Conduct". It's a placebo button. Report them for two things that are actually relevant, or if there's a choice between 'unsportsmanlike' and 'intentional feeding', go for 'intentional feeding'.

2: Attach text. Quote the most toxic thing they said VERBATIM. Quote the phrase that made it clear they were intentionally feeding. If their heaviest toxicity was during champ select, MENTION IT- Riot CAN pull champ select logs, but it isn't done by the automated system- you have to get the report reviewed by human eyes so that this can be done manually.

3: Anything hateful falls under "Hate Speech". Use the Hate Speech box wherever relevant. Seriously, the report will just be ignored if it doesn't reach Riot's standard of 'Hate Speech', but otherwise.. This is the speeding truck of reports. If a player is guilty of this one, and you report it with text in the box (preferably an exact quote), it's a 90% chance they're going to get flagged.
